If you are aged under 65 years and have a permanent and significant disability, you
may be eligible for the National Disability Insurance Scheme (NDIS) services.
The NDIS provides support for people with a disability to achieve their goals,
participate in their community, and be part of the workforce. We provide a range of
supports, either through direct services or by partnering with other providers. By
doing this, we can be a single point of contact for you.
Our diverse range of NDIS services enables us to adjust your supports as your needs
change.

living a quality and meaningful life.
We provide person-centred care supports making us more responsive and effective
than other providers. Whatever your situation, we can offer you the support you
need. We will be supportive, responsive, adaptive and innovative in your choice of
care and lifestyle.
You will have direct access to your dedicated Team Leader who will manage your
support choices.
We can help you get the best out of your new or existing NDIS plan. Your support
plan is individually designed with your health and social goals in mind. Our process is
consultative, and we strive to deliver supports that goes above and beyond.
SUPPORT PROVISION IN THE FOLLOWING AREAS:

you a Team Leader that organises the same, regular team members for your
supports. Our diverse range of services enables us to adjust your support plan as
your needs change.
Whatever your situation, we can offer you the support you need. We will be
supportive, responsive, adaptive, and innovative to suit your choice of care and
lifestyle. You will have direct access to your dedicated Team Leader who will manage
your support choices. Homelife listens to your needs, we care about you and will
provide you with choice and control to build your independence.
